Senior Services Program Intern at Center on Halsted

Essential Functions:
  • Provide case management and facilitate referrals with partner agencies and coordinate programming to meet the needs of the patrons served through the Senior Services Program of Center on Halsted.
Job Description

Duties:
  • Provide one-on-one case management to connect residents with city, state and federal programs
  • Connect patrons to other Chicago agencies and organizations for services
  • Conduct follow-up to ensure referrals have been utilized and were successful
  • Promote financial stability and financial knowledge through workshops, seminars and referrals
  • Assist in making medical, mental health and substance abuse treatment referrals
  • Provide assistance and referrals to patrons in need of housing
  • Develop and implement a comprehensive wellness model of programming to meet the physical, educational, recreational and social needs of the patrons
  • Other duties as assigned

  • Program Development – ability to design programs to meet the needs and interests of the patrons
  • Results – proven track record of exceeding goals and bottom-line orientation; evidence of the ability to consistently make good decisions through a combination of analysis wisdom, expereince, and judgement and the ablity to balance the delivery of programs against the realities of a budget; program solving, and creative resourcesfulness
  • Strategic Vision and Agility – ablity to think strategically, anticipate future consequences and trends, and incorporate them into the organizational plan
  • Action Oriented – enjoys working hard and looks for challenges; able to act and react as necessary, even if limited information is available; not afraid to take charge of a situation
  • Time Management- ability to manage multiple projects simultaneously and to meet deadlines
  • Personal qualities of integrity, credibility, and commitment to the mission of Center on Halsted
  • Lesbian, gay bisexual, transgender sensitivity – demonstrates proficiency on the unique needs of LGBT older adults
  • HIV awareness – previous experience working the HIV-positive community and training on HIV transmission and prevention helpful
  • Communication skills – excellent verbal and written ability; bilingual skills (Spanish / English ) highly desirable
·        Bachelor’s degree in human services or related field at minimum, Master’s degree in social work or related field preferred
